The Inglorious Bastards (Quel maleditto treno blindatto; Counterfeit Commandos; Deadly Mission; G.I. Bro; Hell's Heroes) (1978) [Lt. Robert Yeager]: Shot in the chest by a German soldier on the train; he manages to re-activate the bomb to blow up the train as he dies.

The Kill Reflex (Soda Cracker) (1989) [Ivan Moss]: My memory of this movie is vague, but I believe he was shot to death in a shoot-out with Fred Williamson.

Kill Bill: Vol. 1 (2003) [Reverend Harmony]: Appears as one of the dead bodies in the wedding chapel; all the bodies are shown from a distance, and Bo isn't identified or credited until Vol. 2. (Thanks to Tal and Tommy)

Kill Bill: Vol. 2 (2004) [Reverend Harmony]: Shot to death (off-camera), along with everybody else in the chapel (except for Uma Thurman, who narrowly survives) by the Deadly Viper Assassination Squad; we only hear the shots from outside the chapel. (Thanks to Tal and Tommy)

Inglourious Basterds (2009) [American Colonel]: In the film-within-the-film sequence "Nation's Pride," he is presumably shot to death (off-screen) by Daniel Bruhl. Although it's unconfirmed whether or not Bo survives the battle (in the "complete" version of "Nation's Pride" included in the US DVD, we last see Bo trying to radio in a surrender), I'm listing this just in case.
